MyChart Terms and Conditions of Use

 

Pine Rest Christian Mental Health Services (“Pine Rest”) is pleased to offer you patient information via a Web-based version of our computer system called MyChart.

A MyChart account can be created by any patient age 14 and older. By creating a MyChart account, you indicate that you have read and agree with this MyChart Terms and Conditions of Use statement.

We believe it is important for you to know how we handle information we communicate via MyChart. This MyChart Terms and Conditions of Use outlines our practices and our sensitivity to your right to privacy. Your continued use of MyChart indicates your agreement to be bound by the terms and conditions set forth below.

Privacy

Your privacy is of the utmost importance to us. Pine Rest will use your confidential medical and mental health information in order to provide your health care services. Pine Rest will, at all times, maintain your confidential medical and mental health information in strict confidence and will not disclose any information regarding you to anyone else unless you authorize that person to receive your information  or the information is permitted to be disclosed by law. Pine Rest’s Notice of Privacy Practices provides a thorough description of how we gather, use and protect your confidential information. All messages sent and received within MyChart that contain medical and mental health information are subject to all state and federal laws governing the security and confidentiality of medical records.

Secure Communications
Pine Rest affords the same degree of confidentiality to medical and mental health information stored on MyChart as is given to medical and mental health information stored by Pine Rest in any other medium. Pine Rest is committed to protecting the confidentiality of your medical and mental health information. We limit Pine Rest employees’ access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information.

All communications between you and Pine Rest providers using MyChart occur over a secure connection. You will receive email messages notifying you that new information is available in your MyChart account. These email messages will not contain any confidential medical or mental health information. Although no confidential medical and mental health information will be sent, the notification that new information is available by accessing MyChart may be information that you would not want others to know—please take this into account when providing an email address.

If you send us a message through MyChart, it may be shared with the Pine Rest staff that assists your provider in providing your care. Your confidential medical and mental health information in MyChart will be accessible only to appropriate Pine Rest staff.

MyChart should never be used for urgent matters. If you have a life threatening medical or mental health emergency situation, please call 9-1-1. Pine Rest will make its best effort to provide a timely response to electronic inquiries. Please allow at least 3 business days for a response.

Access Code
When you initially enroll in MyChart, you will be provided with an access code. The access code you receive will give you the opportunity to create a personal login name (called “MyChart Username”) and a password. Once this access code has been used it will no longer be valid. It is extremely important that you keep your username and password completely confidential. Anyone with access to your username and password will be able to access your confidential medical and mental health information.  That person will also be able to read your messages and send new messages as if they were you. It is your responsibility to prevent disclosure of your username and password, and to change your password if you feel that your security has been compromised. You may change your password at any time.

Proxy Access
A patient may grant proxy access to his/her MyChart account by completing a Proxy Access MyChart Consent Form in person at any Pine Rest location. The patient will be able to allow full access to the MyChart (including clinical information) or Scheduling/Billing only.

Minor Patient Proxy Account
Parents or guardians of minor children may request access to the minor child's account. Such access will only be granted to parties with parental rights or legal guardianship over the minor patient and only to the extent that the party requesting proxy access can demonstrate the legal right to patient’s medical information (including mental health).  To comply with federal and state regulations, proxy accounts for minor children between the ages of 12 and 17 (inclusive) will have limited access to clinical information in MyChart. However, MyChart tools regarding scheduling, billing, messaging, and prescription refill requests will be accessible. Clinical information can be requested through standard release of information processes at any Pine Rest location.
If the proxy’s legal relationship with the account holder changes, the proxy must inform Pine Rest immediately by calling (616) 281-6349 (or toll free (800) 678-5500, ext. 5943) or by sending written notice to Medical Records Director:  300 68th Street SE, PO Box 165, Grand Rapids, MI 49501-0165. Pine Rest reserves the right to revoke proxy access at any time for any reason.

Adult Legal Guardianship Proxy Account
Legal guardians of adult patients may request access to the patient’s MyChart account, giving full access, including clinical information. Such access will only be granted to parties with legal guardianship over the patient and only to the extent that the party requesting proxy access can demonstrate the legal right to patient’s medical information (including mental health).  If the proxy’s legal relationship with the account holder changes, the proxy must inform Pine Rest immediately by calling (616) 281-6349 (or toll free (800) 678-5500, ext. 5943) or by sending written notice to Medical Records Director:  300 68th Street SE, PO Box 165, Grand Rapids, MI 49501-0165. Termination of Access
Pine Rest may restrict or terminate access to MyChart at any time.  If access is restricted for planned maintenance of the MyChart system, every effort will be made to communicate downtime to all users in advance.  However, there may be reasons to restrict access to MyChart without advance notice.  In addition, if your use of MyChart violates these Terms and Conditions, Pine Rest reserve the right to permanently terminate access to MyChart.

Restrictions on Use and Ownership
All pages within MyChart and any material made available for download are the property of Pine Rest.  The content on the Site is provided for Pine Rest Providers and its patients only.  The words “Pine Rest” and its associated corporate logo are the trademarks and service marks of Pine Rest Christian Mental Health Services.  All other trademarks, service marks, and logos used in this Site, including but not limited to MyChart and Epic, are the trademarks, service marks, or logos of their respective owners.  Nothing on this Site should be construed as granting any license or right to use any trademark, service mark, or logo displayed on this Site without the express permission of Pine Rest or the third party that may own the trademark, service mark, or logo. 

Accuracy and Integrity of Information
Although Pine Rest attempts to ensure the integrity of the Site, occasionally it is possible that the Site could include typographical errors, inaccuracies or other errors. In the event that an inaccuracy arises, please inform Pine Rest so that it can be corrected. Information contained on the Site may be changed or updated without notice.

MyChart may offer links to related medical Web sites not managed by Pine Rest. These Web site link(s) are for a patient’s informational purposes only. Pine Rest does not endorse and has not verified the accuracy of the information in/on these Web sites, and the patient should not rely on any of the information found on the Web sites for purposes of treatment or diagnosis.
